Purpura in traditional Chinese medicine is mainly related to factors such as excessive blood heat, lack of qi to absorb blood, yin deficiency and excessive fire, spleen not regulating blood, and blood stasis obstructing collaterals. Treatment should adopt principles such as clearing heat and cooling blood, supplementing qi and blood, nourishing yin and reducing fire, strengthening spleen and regulating blood, and promoting blood circulation and removing blood stasis according to the syndrome type. Purpura is a hemorrhagic disease characterized by the appearance of petechiae and ecchymoses on the skin and mucous membranes. Traditional Chinese medicine believes that its onset is closely related to dysfunction of the organs and abnormal circulation of qi and blood.

1. Bloody fever and reckless behavior
are often caused by external heat pathogens or emotions turning into fire, leading to hot blood and forcing blood to flow recklessly and overflow outside the pulse. Patients often have dense distribution of bright red purpura, accompanied by symptoms such as thirst, dry throat, red tongue, and yellow coating. The rhinoceros horn rehmannia decoction can be added or subtracted. traditional Chinese patent medicines and simple preparations such as lotus leaf pills, shicao oil capsules, and heat clearing rehmannia pills have the effect of cooling blood and stopping bleeding. Treatment should avoid spicy and hot foods and maintain emotional stability.
2. Qi does not absorb blood
Due to prolonged illness or excessive fatigue, Qi deficiency cannot absorb blood, often resulting in repeated attacks of light purple bruises, accompanied by shortness of breath and fatigue. Guipi Decoction is commonly used in combination with qi tonifying drugs such as astragalus and ginseng, and traditional Chinese patent medicines and simple preparations such as Buzhong Yiqi Granule, Guipi Pill, Shengmai Drink, etc. Daily attention should be paid to avoiding intense exercise, and spleen strengthening ingredients such as yam and red dates can be consumed appropriately.
3. Yin deficiency and excessive fire
Yin deficiency in the liver and kidneys leads to internal disturbance of deficiency fire, and blood overflows with the movement of fire, manifested as purpura with dark red color accompanied by hot flashes and night sweats. Zhibai Dihuang Pill, Erzhi Pill and other Chinese patent medicines such as Liuwei Dihuang Pill, Zuogui Pill and Dabuyin Pill were used to nourish yin and reduce fire. Sexual activity should be restrained, and staying up late should be avoided. Yin nourishing products such as Tremella fuciformis and black sesame seeds can be consumed.

4. Spleen failure to regulate blood
Spleen deficiency caused by lack of regulation results in purpura with a pale red color, often accompanied by dental bleeding and nosebleeds with loss of appetite. The prescription uses Huangtu Decoction plus or minus, traditional Chinese patent medicines and simple preparations such as Ginseng Jianpi Pill, Shenling Baizhu Powder, Sijunzi Granule, etc. Diet should be gentle and easy to digest, avoiding raw, cold, and greasy foods. It can be combined with moxibustion at acupoints such as Zusanli.
5. Blood stasis obstructing collaterals
Prolonged illness or trauma leading to blood stasis obstruction, purpura appearing purplish blue and fixed in position. It is commonly used to add or subtract Xuefu Zhuyu Decoction, and traditional Chinese patent medicines and simple preparations such as Compound Danshen Tablets, Xuefu Zhuyu Capsules, Sanqi Powder, etc. It can cooperate with acupuncture and moxibustion and massage to promote the movement of qi and blood and avoid sitting and lying for a long time. Patients with purpura should keep their skin clean and avoid scratching, and their clothing should be loose and soft. A balanced diet should be maintained, with plenty of fresh vegetables and fruits rich in vitamin C, and avoiding foods such as lamb and chili peppers. During an acute attack, one should rest in bed and observe the bleeding situation. If there are signs of visceral bleeding, seek medical attention immediately. During the treatment period, regularly check the blood routine and cooperate with the doctor to adjust the medication plan. Do not stop using blood activating or hemostatic drugs on your own. Traditional Chinese medicine conditioning should be persisted for 3-6 months, combined with soothing exercises such as Ba Duan Jin and Tai Chi to improve qi and blood circulation.
