How to treat lichen planus and root it out best

Lichen planus can be treated comprehensively through external application of glucocorticoids, immune modulators, phototherapy, traditional Chinese medicine regulation, and lifestyle habits adjustment. Lichen planus is a chronic inflammatory skin disease that may be related to immune abnormalities, genetic factors, viral infections, mental stress, and drug stimulation.

1. Topical use of glucocorticoids

Corticosteroid ointments such as hydrocortisone butyrate cream and mometasone furoate cream can alleviate skin inflammation and itching. Suitable for localized skin lesions, short-term use under the guidance of a doctor to avoid side effects such as skin atrophy. Combining with moisturizers can reduce local irritation.

2. Immunomodulators

Tacrolimus ointment, Pimecrolimus cream, and other calcineurin inhibitors are suitable for the face or thin and tender skin areas, and can regulate local immune responses. Long term use has high safety, but attention should be paid to the possible burning sensation in the initial stage.

3. Narrow spectrum medium wave ultraviolet phototherapy

NB-UVB phototherapy improves extensive skin lesions by inhibiting T cell activity, 2-3 times a week, and lasts for several months. During treatment, it is necessary to strengthen skin moisturizing and avoid the use of photosensitive drugs. Local PUVA therapy may be considered for stubborn oral lichen planus.

4. Traditional Chinese Medicine Syndrome Differentiation and Regulation

For blood heat type, heat clearing and blood cooling formulas such as modified Xijiao Dihuang Decoction can be used. For blood stasis type, Taohong Siwu Decoction is selected. For yin deficiency type, Zhibai Dihuang Pill is commonly used. For external treatment, decoction of Huangbai, Sophora flavescens and other herbs can be used for wet compress, or Qingdai powder can be mixed with oil for application. Traditional Chinese medicine practitioners need to diagnose and treat based on syndrome differentiation.

5. Adjusting lifestyle habits

Avoid scratching and rubbing skin lesions, and choose breathable cotton clothing. Reduce spicy and stimulating diet, quit smoking and limit alcohol consumption. Maintain a regular schedule to relieve mental stress. Patients with oral problems should use a soft bristled toothbrush to avoid stimulating the mucous membranes with acidic foods.

Long term standardized treatment and regular follow-up to evaluate the efficacy of lichen planus are necessary. Daily attention should be paid to moisturizing and sun protection of the skin, and patients with oral lesions should undergo mucosal examinations every six months. Traditional Chinese medicine conditioning can be combined with ear acupressure, acupoint injection and other auxiliary therapies. The diet should be light and rich in vitamins, and moderate exercises such as Ba Duan Jin should be practiced to regulate immune balance. If the skin lesions suddenly worsen or ulceration occurs, seek medical attention promptly.
