What should I do if I experience lower back pain 4 days after ureteral lithotripsy

Lower back pain that occurs 4 days after ureteral lithotripsy can be treated through bed rest, hot compress, appropriate drinking water, medication for pain relief, and timely medical attention. Lower back pain may be related to factors such as residual stones, damage to the ureteral mucosa, urinary tract infections, hydronephrosis, and postoperative spasms.

1. Bed rest

Early postoperative advice is to reduce activity and adopt a lateral or semi recumbent position to relieve lumbar pressure. Avoid sudden twisting or bending movements to prevent stone movement from irritating the ureter. It can be supported by a soft pillow to elevate the waist and maintain a comfortable position.

2. Heat compress relief

Apply a hot towel at around 40 ℃ to the painful area for 15-20 minutes each time. Hot compress can relax the smooth muscles of the ureter and relieve spasmodic pain. Be careful to avoid burning the skin, and repeat 3-4 times a day.

3. Drink water appropriately

Maintain a daily water intake of 2000-2500 milliliters to increase urine output and help eliminate residual debris. It is recommended to drink warm water in small amounts in batches to avoid excessive drinking in a short period of time, which can increase the burden on the kidneys. Observe whether the urine is cloudy or has blood streaks.

4. Medication analgesia

If the pain is obvious, follow the doctor's advice to use analgesics such as diclofenac sodium suppositories, tramadol hydrochloride sustained-release tablets, or ibuprofen capsules. When co infected, antibiotics such as cefaclor dispersible tablets and levofloxacin tablets should be used for treatment.

5. Seek medical attention in a timely manner

If there is persistent severe back pain with fever, worsening hematuria, or anuria, immediate follow-up of urinary tract CT is necessary. There may be significant stone impaction, severe infection, or ureteral perforation, and if necessary, secondary surgery or placement of double-J tube drainage may be necessary. After surgery, the perineum should be kept clean and loose cotton underwear should be worn to avoid friction. Choose low salt and low-fat Congee, steamed fish and other digestible foods, and limit spinach, tofu and other high oxalate foods. Within two weeks, vigorous exercise and heavy physical labor are prohibited, and regular ultrasound examinations are conducted to observe the stone removal situation. If there is recurrent pain or abnormal urination, the attending physician should be contacted promptly to adjust the treatment plan.
