High percentage of lymphocytes in physical examination

The high percentage of lymphocytes in physical examination may be a normal physiological phenomenon, and may also be related to virus infection, tuberculosis, immune system diseases, etc. It needs to be comprehensively evaluated in combination with clinical manifestations and other examinations. When a health examination reveals a mild increase in the proportion of lymphocytes without any other abnormalities, it is common in infants and young children with physiological lymphocytosis or in adults in the recovery period from recent viral infections. The increase in lymphocytes caused by viral infection is usually accompanied by upper respiratory symptoms such as fever and sore throat, and blood tests may also show synchronous increase in monocytes, which is self limiting. Subclinical infections caused by certain viruses such as EB virus and cytomegalovirus can also lead to sustained high lymphocyte ratios without typical symptoms. Within 2-4 weeks after partial vaccination, there may be an increase in lymphocyte reactivity, which is related to immune response related lymphocyte clonal proliferation. What requires clinical attention is a sustained and significant increase in the proportion of lymphocytes. A large number of mature lymphocytes can be seen in peripheral blood smears of patients with chronic lymphocytic leukemia, and bone marrow examination shows clonal proliferation of lymphocytes. The activation of T lymphocytes in patients with tuberculosis caused by the stimulation of TB bacilli, in addition to the increased proportion of lymphocytes, is often accompanied by rapid ESR and positive tuberculin test. Autoimmune diseases such as active rheumatoid arthritis can also reflect lymphocyte activation caused by inflammatory cytokine stimulation in blood routine tests. Long term use of certain drugs such as phenytoin sodium and carbamazepine may cause drug-induced lymphocytosis.

It is recommended to complete peripheral blood cell morphology examination and screen for abnormal lymphocytes when abnormal lymphocyte proportion is found. Patients with persistent symptoms such as fatigue and weight loss need to undergo in-depth examinations such as lymph node ultrasound and bone marrow puncture. In daily life, it is important to avoid staying up late and ensure high-quality protein and vitamin intake to aid in immune regulation. Avoid using drugs or health supplements that regulate immunity without authorization, and all therapeutic interventions should strictly follow the guidance of hematologists or rheumatologists.
