What is the reason for high levels of creatine kinase in myocardial enzymes

Elevated myocardial enzyme creatine kinase may be caused by intense exercise, muscle injury, myocarditis, myocardial infarction, drug side effects, etc. It can be improved through rest observation, symptomatic treatment, thrombolytic therapy, and discontinuation of relevant drugs.

1. Intense exercise:

High intensity exercise can cause slight damage to skeletal muscle cells, releasing creatine kinase into the bloodstream. After anaerobic exercises such as marathons and strength training, creatine kinase can increase to more than 5 times the normal value and gradually recover within 72 hours. It is recommended to replenish electrolytes after exercise and avoid intense physical activity.

2. Muscle injury:

Trauma, compression injury, or muscle injection may cause rhabdomyolysis, with creatine kinase levels reaching thousands to tens of thousands of U/L. Accompanied by myoglobinuria, it may cause acute kidney injury. A large amount of fluid replacement is needed to alkalize urine, and in severe cases, blood purification treatment is required. 3. myocarditis: After viral infection, the immune response can damage myocardial cells, with creatine kinase isoenzyme CK-MB accounting for more than 5%. Patients often have a history of pre infection, manifested as chest tightness, palpitations, and ST-T changes on electrocardiogram. The treatment includes bed rest and nutritional myocardial drugs such as coenzyme Q10.

4. Myocardial infarction: When coronary artery occlusion leads to myocardial necrosis, creatine kinase starts to increase at 4-6 hours and reaches its peak at 18-24 hours. The typical symptoms are persistent chest pain accompanied by sweating, and pathological Q waves can be seen on the electrocardiogram. Urgent coronary angiography is required, and if necessary, stent implantation surgery may be performed.

5. Drug side effects:

Statins as lipid-lowering drugs may cause drug-induced myopathy, with creatine kinase levels exceeding 10 times and requiring discontinuation of medication. Antipsychotic drugs such as clozapine and immunosuppressant cyclosporine can also cause muscle enzyme abnormalities. Any abnormalities should be promptly rechecked, and if necessary, the medication regimen should be changed.

The elevation of creatine kinase should be judged based on clinical symptoms, and mild elevation without discomfort can be re examined one week later. Avoid excessive fatigue in daily life and ensure high-quality protein intake to promote muscle repair. Hypertension patients need to control their blood pressure to reach the standard, and diabetes patients need to maintain stable blood sugar. If symptoms such as persistent chest pain, oliguria, or muscle weakness occur, immediate medical attention should be sought to investigate serious illnesses. Regular physical examinations are recommended to include tests such as troponin and electrocardiogram to assist in differential diagnosis.
