Shoulder periarthritis can be treated through hot compress therapy, massage, acupuncture therapy, external application of traditional Chinese medicine, functional exercise, and other methods. Shoulder periarthritis is usually caused by invasion of wind cold and dampness pathogens, long-term fatigue, insufficient qi and blood, external injury and blood stasis, liver and kidney deficiency, and other reasons.
1. Hot compress therapy
uses moxa stick suspension moxibustion or traditional Chinese medicine hot compress packs for local hot compress, 1-2 times a day, which can promote local blood circulation. Wind cold type shoulder periarthritis can be treated with Qianghuo Shengshi decoction for reducing fumigation and washing. The temperature for wet and hot compress should be controlled at 50-60 ℃, and care should be taken to avoid burning the skin. Combined with infrared irradiation, the effect is better, with each irradiation lasting 20 minutes.
2. Tuina massage
uses techniques such as rolling, kneading, and holding to loosen shoulder adhesions, with a focus on massaging acupoints such as the shoulder spine and shoulder ridge. During the acute phase, mild manipulation is recommended to relieve pain, while during the chronic phase, the intensity of manipulation can be increased. It is recommended to have a professional physician perform the massage. When massaging on your own, you can use Huoluo oil to assist, but avoid violent joint movements.
3. Acupuncture therapy
Select the shoulder three needles as the main acupoints, namely Shoulder Hair, Shoulder Liao, and Shoulder Zhen, combined with distal acupoints such as Tiaokou and Yanglingquan. Cold dampness type plus moxibustion method, blood stasis type using needling and cupping. Treat 3 times a week, with 10 times as one course of treatment. Electroacupuncture therapy has a significant effect on relieving muscle spasms, but it should be used with caution in patients with heart disease.
4. External application of traditional Chinese medicine
In the acute phase, use Jinhuang San to adjust vinegar for external application to reduce swelling and relieve pain, and in the chronic phase, use Guizhi and Gegen decoction residue for hot compress. Traditional Chinese patent medicines and simple preparations plasters such as dampness relieving pain plaster and pain relieving plaster can be selected. Plasters containing ingredients such as Sichuan aconite and kusnezoff root are forbidden for pregnant women. People with skin allergies should stop using it immediately, and the external application time should not exceed 8 hours.
5. Functional exercises
Climbing walls, pendulum movements, etc. are gradually carried out, with 3 groups per day, 10-15 times per group. During the pain relief period, resistance training with elastic bands can be performed, but excessive abduction movements should be avoided. Swimming is a good whole-body rehabilitation exercise, and the water temperature should be kept above 28 ℃ to prevent catching a cold.
During treatment, pay attention to keeping your shoulders warm, avoid lifting heavy objects, and avoid excessive use of air conditioning. Diet should be warm and nourishing, and moderate consumption of cold dispersing ingredients such as lamb and ginger can be recommended. Avoid eating raw and cold foods. Keep the affected side pillow in a functional position during sleep, and take nonsteroidal anti-inflammatory drugs for a short period of time when the pain is severe. If conservative treatment is ineffective for 3 months, interventional treatment such as intra-articular injection or release surgery should be considered.
